ABSTRACT

Over the last decade, Brazil registered a 10.56% increase in the number of poultry slaughtered in establishments registered under the "Serviço de Inspeção Federal" (SIF - Federal Inspection Service), as a result of technological advances in management, health and genetics applied to national aviculture. At slaughter, during post-mortem inspection, carcasses can be totally or partially condemned for various reasons, including ascitic syndrome. This syndrome has economic implications for the industry, in addition to being a problem for the health and welfare of poultry. The objective of this work was to evaluate the historical series (2010-2019) of partial and total condemnations of poultry carcasses due to ascitic syndrome in slaughterhouses registered under the SIF and located in the main poultry-producing states. Through official data, the condemnation occurrence index (COI) and the adjusted seasonal index (ASI) were calculated. The condemnation rate was 1,140 carcasses condemned, totally or partially, for ascitic syndrome for every 1,000,000 poultry slaughtered. The smallest and largest COIs were found in São Paulo (February 2010) and in Goiás (January 2017), respectively. The occurrence of condemnations for this syndrome was cyclical throughout the historical series, showing peaks of condemnation in all years evaluated, with the highest ASIs in July, August and September, and with rates varying between 1.24 and 1.54 in these months. Considering the period of pre-slaughter housing, the highest ASIs coincide with the coldest period of the year (May to August) for all analyzed states. The results show that ascitic syndrome is a growing problem in Brazil, with greater occurrence during the coldest months of the year, having a negative impact on animal health and the profitability of producers and industries in the poultry production chain.(AU)


O objetivo deste trabalho foi avaliar a série histórica (2010-2019) de condenações parciais e totais de carcaças de aves por síndrome ascítica em abatedouros registrados no SIF localizados nos principais estados produtores de carne de frango. Através de dados oficiais, foram calculados o índice de ocorrência de condenações (IOC) e o índice de sazonalidade ajustado (ISA). A taxa de condenação foi de 1.140 carcaças condenadas total ou parcialmente por síndrome ascítica a cada 1.000.000 de aves abatidas. O menor e maior IOCs foram encontrados em São Paulo (fevereiro/2010) e em Goiás (janeiro/2017), respectivamente. A ocorrência de condenações por esta síndrome teve comportamento cíclico ao longo da série histórica, apresentando picos de condenação em todos os anos avaliados, sendo os maiores ISAs encontrados nos meses de julho, agosto e setembro, com índices variando entre 1,24 e 1,54 nestes meses. Considerando o período de alojamento pré-abate, os maiores valores de ISAs coincidiram com o período mais frio do ano (Maio a Agosto) para todos os estados analisados. Os resultados mostram que síndrome ascítica é um problema crescente no Brasil, com maior ocorrência durante os meses frios do ano, impactando negativamente a saúde dos animais e a lucratividade dos produtores e indústrias da cadeia produtiva de frango de corte.(AU)

ABSTRACT

ABSTRACT Hepatitis A and hepatitis E are the leading causes of acute viral hepatitis in developing countries due to our poor sanitary conditions, both spread by fecal-oral route or through contaminated water and food. Being both self-limiting diseases, they are usually benign but may present with atypical clinical findings. A 32 year-old female with right pleural effusion, ascites and acalculous cholecystitis during the course of HAV and HEV co-infection is reported. Clinical improvement was observed with conservative management. As far as we know, this is the first case described of a patient with these three complications in the background of a hepatitis A virus and hepatitis E virus co-infection.


RESUMEN Hepatits A y hepatitis E son las principales causas de hepatitis viral en países en desarrollo debido a las limitadas condiciones sanitarias. Son condiciones usualmente benignas y autolimitadas, pero pueden presentarse de forma atípica. Se reporta una paciente de 32 años con efusión pleural derecha, colecistitis acalculosa y ascitis en el curso de una co-infección por el virus de Hepatitis A y hepatitis E. Hasta donde tenemos conocimiento, este es el primer caso de una paciente con estas tres complicaciones como resultado de una infección por el virus de hepatitis A y hepatitis E.

Abstract The occurrence of ascites after Renal Transplant (RT) is infrequent, and may be a consequence of surgical or medical complications. Case report: 61 year-old, male, history of arterial hypertension, tongue carcinoma and alcoholic habits 12-20g/day. He had chronic kidney disease secondary to autosomal dominant polycystic kidney disease, without hepatic polycystic disease. He underwent cadaver donor RT in September 2017. He had delayed graft function by surgically corrected renal artery stenosis. He was admitted in January 2018 for ascites de novo, with no response to diuretics. HE had visible abdominal collateral circulation. Graft dysfunction, adequate tacrolinemia, Innocent urinary sediment, mild anemia, without thrombocytopenia. Serum albumin 4.0g / dL. Normal hepatic biochemistry. Peritoneal fluid with transudate characteristics and serum albumin gradient > 1.1. Ultrasound showed hepatomegaly, permeable vascular axes, without splenomegaly. Mycophenolate mofetil was suspended, with reduced remaining immunosuppression. He maintained refractory ascites: excluded infectious, metabolic, autoimmune and neoplastic etiologies. No nephrotic proteinuria and no heart failure. MRI: micronodules compatible with bile cysts. Upper Digestive Tract Endoscopy did not show gastroesophageal varicose veins. Normal abdominal lymphoscintigraphy. He underwent exploratory laparoscopy with liver biopsy: incomplete septal cirrhosis of probable vascular etiology some dilated bile ducts. He maintained progressive RT dysfunction and restarted hemodialysis. The proposed direct measurement of portal pressure was delayed by ascites resolution. There was further recovery of the graft function. Discussion: Incomplete septal cirrhosis is an uncommon cause of non-cirrhotic portal hypertension. Its definition is not well known, morphological and pathophysiological. We have not found published cases of post-RT ascites secondary to this pathology, described as possibly associated with drugs, immune alterations, infections, hypercoagulability and genetic predisposition.


Resumo A ocorrência de ascite no pós-Transplante Renal (TR) é infrequente, podendo ser consequência de complicações cirúrgicas ou médicas. Caso clínico: 61 anos, masculino, antecedentes de hipertensão arterial, carcinoma da língua e hábitos alcoólicos 12-20g/dia. Doença renal crônica secundária à doença renal poliquística autossômica dominante, sem poliquistose hepática. Submetido a TR de doador cadáver em setembro de 2017. Atraso na função de enxerto por estenose da artéria renal, corrigida cirurgicamente. Internado em janeiro de 2018 por ascite de novo, sem resposta a diuréticos. Circulação colateral abdominal visível. Disfunção do enxerto, tacrolinemia adequada. Sedimento urinário inocente. Anemia ligeira, sem trombocitopenia. Albumina sérica 4,0g/dL. Bioquímica hepática normal. Líquido peritoneal com características de transudado e gradiente sero-ascítico de albumina > 1,1. Ecografia com hepatomegalia, eixos vasculares permeáveis, sem esplenomegalia. Suspendeu micofenolato mofetil, reduziu restante imunossupressão. Manteve ascite refratária: excluídas etiologias infecciosas, metabólicas, autoimunes e neoplásicas. Sem proteinúria nefrótica e sem insuficiência cardíaca. RM: micronódulos compatíveis com quistos biliares. EDA sem varizes gastroesofágicas. Linfocintigrafia abdominal normal. Submetido a laparoscopia exploradora com biópsia hepática: cirrose septal incompleta de provável etiologia vascular, alguns ductos biliares dilatados. Manteve disfunção progressiva do TR, reiniciou hemodiálise. Proposta medição direta da pressão portal, protelada por resolução da ascite. Recuperação posterior da função de enxerto. Discussão: A cirrose septal incompleta é uma causa incomum de hipertensão portal não cirrótica. A sua definição é morfológica e a fisiopatologia, pouco conhecida. Não encontramos publicados casos de ascite pós-TR secundária a esta patologia, descrita como possivelmente associada a fármacos, alterações imunitárias, infecções, hipercoagulabilidade e predisposição genética.

ABSTRACT

INTRODUCCIÓN La peritonitis meconial (PM) es una peritonitis localizada o generalizada, aséptica, química o de cuerpo extraño; producto del paso de meconio a la cavidad peritoneal y esta correlacionada con la perforación prenatal del tracto digestivo. Se presenta en 1 de cada 30.000 recién nacidos (RN). El diagnostico ecográfico prenatal mejora los resultados perinatales, el hallazgo más frecuente es la ascitis. La PM requiere un tratamiento multidisciplinario urgente, la mayoría es de resolución quirúrgica. OBJETIVO Dar a conocer una patología infrecuente, que requiere un alto grado de sospecha diagnostica para otorgar un manejo perinatal específico y oportuno. Caso clínico Primigesta de 29 años sin antecedentes mórbidos, cursando embarazo controlado de 36+5 semanas. En control ecográfico se evidencia ascitis fetal. Se hospitaliza en alto riesgo obstétrico, descartando patología metabólica e infecciosa. A las 37 semanas por cesárea de urgencia, se obtiene RN con distensión abdominal y hepatomegalia. En laparotomía exploradora se evidencia asas intestinales indemnes. En re intervención se encuentra hernia de íleon distal perforada, se confecciona ostomia, evoluciona favorablemente y es dado de alta. DISCUSIÓN Es fundamental considerar la PM dentro de los diagnósticos diferenciales de ascitis fetal. Un diagnóstico oportuno mejora los resultados perinatales y permite prevenir posibles complicaciones.


INTRODUCTION Meconial peritonitis (PM) is localized or generalized peritonitis, aseptic, chemical or strange body; a product of meconium steps to the peritoneal cavity and is correlated with prenatal perforation of the digestive tract. It occurs in 1 in 30,000 newborns. Prenatal ultrasound diagnosis improves perinatal outcomes, the most frequent finding is ascites. PM requires urgent multidisciplinary treatment, most of cases need surgical resolution. OBJECTIVE To present an infrequent pathology, which requires a high degree of diagnostic suspicion to grant a specific and timely perinatal management. Clinical case Pregnant 29 years old woman without morbid history, 36 weeks of controlled pregnancy. Ultrasound control shown fetal ascites. He is hospitalized at high obstetric risk, ruling out metabolic and infectious pathology. At 37 weeks by emergency caesarean section, is obtained a baby boy with abdominal distension and hepatomegaly. In exploratory laparotomy there are undamaged intestinal handles. In re intervention it is noted herniated perforated distal ileum, ostomy is made. Patient evolves favourably and is discharged. DISCUSSION It is essential consider PM within the differential diagnoses of fetal ascites. An opportune diagnosis improves the perinatal results and could avoids possible complications.

BACKGROUND: The aim of this study was to assess the role of transabdominal ultrasonography in characterizing and determining the etiology of ascites in comparison with laboratory ascitic fluid analysis and other methods used to establish the final diagnosis. METHODS: A prospective descriptive study was conducted on 61 patients with ascites attending outpatient department (OPD) or admitted to wards of Tikur Anbesa Specialized Hospital (TASH) and referred to radiology department for imaging from June 2017 to November 2017. Data were collected following the internationally recommended scanning technique in consecutive bases. The data were analyzed using SPSS version 20. The comparison of ultrasound and laboratory findings with final clinical diagnosis was analyzed using Chi-square test (X2). RESULTS: Of 61 patients with ascites enrolled in this study, females were 35(57.4%) with age range of 16 to 75 and mean age of 43.2±14.11. The cause of ascites was established in 59 cases using a combination of clinical, pathological, imaging evidences and tumor markers. However there were two cases who had ascites with indeterminate cause. US suggested the diagnosis in 54(91.5%) patients. Excluding mixed and indeterminate cases, ultrasound characterized ascites correctly as exudate and transudate in 95% cases. CONCLUSION: Ultrasound has significant accuracy to distinguish transudate and exudate ascites and in suggesting the underlying cause. It can be a valuable method of investigation of ascites in places where CT and MRI are not available, and it is the best complement for laboratory investigations on ascites in suggesting the etiology based on ascitic fluid texture and ancillary findings

Introduction : Les ascites métastatiques cancéreuses sont caractérisées par la présence de cellules cancéreuses métastatiques dans le liquide péritonéal. Elles sont fréquentes à un stade avancé des cancers. Il s'agissait de déterminer la contribution de la cytologie au diagnostic et à l'évaluation du pronostic des ascites métastatiques dans le service d'Anatomie et Cytologie Pathologiques du CHU du Point G. Matériel et méthode : Notre étude s'est étendue de Janvier 2013 à Décembre 2015; soit une période de 3 ans. Elle était rétrospective et prospective incluant tous les patients dont l'étude cytologique des liquides d'ascitea mis en évidence des cellules malignes. Le dossier médical des patients a été exploité pour la recherche du site primitif et du diagnostic histologique éventuel et le suivi. Résultats : Sur 213 examens cytologiques de liquide d'ascite; 61 étaient malins, soit un taux de 28,64%. La tranche d'âge la plus répresentée était de 46 à 60 ans avec un âge moyen de 53,7 ± 14,7 ans. Le sexe féminin était dominant avec un taux de 66%. La profession la plus fréquente était les ménagères soit un taux de 50,82%. Le siège primitif le plus fréquent était l'ovaire (26%). Il était suivi par le foie et l'estomac avec respectivement 20% et 18%. L'ascite a été le signe révélateur du cancer dans 36% des cas. Elle était isolée dans 78%. Après 6 mois de suivi, le taux de mortalité était 75,6%. Conclusion : L'examen cytologique constitue un examen paraclinique dans le diagnoticdes ascites métastatiques cancéreuses

ABSTRACT Introduction. The aim of this study is to investigate large volume therapeutic paracentesis using either a z-tract or axial (coxial) technique in a randomized controlled trial. Materials and methods. In this randomized, single blind study, patients with cirrhosis undergoing outpatient therapeutic paracentesis were randomized to the z-tract or the modified angular (coaxial) needle insertion technique. Subject and procedure characteristics were compared between the groups with ascites leakage as quantified by need for dressing changes with standardized sized gauze pads as a primary endpoint and subject procedural discomfort, operator preference, and procedure complications as secondary endpoints. Results. 72 paracenteses were performed during the study period: 34 to the z-tract and 38 to the coaxial insertion technique. Following exclusions, a total of 61 paracenteses were analyzed: 30 using the z-tract technique and 31 using the coaxial technique. There were equal rates of post-procedural leakage of ascites between groups (13% in both groups, p = 1.00). Using the visual analog scale (0 - 100), there was a statistically significant increase in the subject reported pain score with the z-tract compared with the coaxial method [26.4 (95% CI 18.7 - 34.1) vs. 17.2 (95% CI 10.6 - 23.8), p = 0.04]. Mean physician rated procedure difficulty (1 - 5) was significantly higher for the z-tract vs. the coaxial technique [2.1 (95% CI 1.6 - 2.6) vs. 1.5 (95% CI 1.2 - 1.8), p = 0.04]. Conclusion. When compared to the z-tract technique, the coaxial insertion technique is superior during large volume paracentesis in cirrhosis patients.

ABSTRACT

La tuberculosis peritoneal es una enfermedad que a nivel mundial tiene una incidencia del 0.5% y representa del 3- 4% de los casos de tuberculosis. Honduras ocupa el octavo lugar en Latinoamérica y el segundo en América Central en los índices de tuberculosis pulmonar y extrapulmonar; no hay estadísticas actualizadas de la incidencia de la variante peritoneal en el país. Caso clínico:paciente femenina de 15 años, con antecedente de ingesta de leche de vaca no pasteurizada, historia de tres meses de febrícula, acompañada de ascitis, amenorrea, pérdida de peso, dolor abdominal y aumento del perímetro del mismo de dos meses de evolución. Se realizó citoquímica de líquido peritoneal que reportó proteínas de 10,950 mg/dl, adenosina deaminasa con resultado positivo de 140.2 U/L, este último tiene alta sensibilidad y especificidad para identificar ascitis por tuberculosis peritoneal. Ultrasonido pélvico y abdominal con resultado dentro de parámetros normales, la tomografía abdominal contrastada reportó ascitis de 3,600 ml, engrosamiento peritoneal y linfadenopatias con colecciones correspondientes a tuberculosis peritoneal. La paciente recibió tratamiento antifímico, posteriormente presentó disminución del perímetro abdominal, resolución de ascitis y aumento de peso, indicando respuesta favorable al tratamiento. Conclusión: la tuberculosis peritoneal respondió eficazmente al tratamiento antifimico empleado para la tuberculosis pulmonar. Identificar un caso nuevo tiene importante valor epidemiológico, sobre todo en áreas rurales donde la población tiende a consumir la leche de vaca no pasteurizada...(AU)

ABSTRACT

Lupus peritonitis (LP) is extremely rare. Acute LP is characterized by rapid onset of ascites and severe abdominal pain, in addition to other well-recognized clinical features of a general systemic lupus erythematosus (SLE) fare. Ascites associated with acute LP has been rarely reported as the prominent feature of a SLE fare. We report a 39-year-old woman who developed massive, painful ascites as the presenting manifestation of a SLE fare. Diagnostic workup ruled out the possibility of hepatic, cardiovascular, infectious, or malignant diseases, and confirmed the presence of a SLE fare. The patient was treated with methyl prednisolone and hydroxychloroquine resulting in dramatic improvement of her condition. During ambulatory follow up, she has remained asymptomatic up to the moment of this writing. Adrenal steroids and hydrocychloroquine may be useful for the management of SLE fares in patients with massive, painful ascites due to acute LP.


La peritonitis lúpica es rara. El cuadro agudo se caracteriza por ascitis de comienzo rápido, dolor abdominal severo y otras manifestaciones de una recidiva de un lupus eritematoso sistémico (LES). Sólo ocasionalmente e ha informado que la ascitis y peritonitis lúpica sean las principales manifestaciones de una recidiva lúpica. Presentamos a una mujer de 39 años que presentó ascitis masiva y dolorosa como la primera manifestación de una reactivación lúpica. El estudio diagnóstico descartó problemas hepáticos, pulmonares, cardiacos, cáncer o infecciones y confirmó la reactivación lúpica. La paciente se trató con metilprednisolona y cloroquina, resultando en una mejoría dramática. Al momento de preparar este informe, la paciente permanece asintomática en control ambulatorio. Los corticoides y cloroquina pueden ser medicamentos útiles para el tratamiento de pacientes con ascitis dolorosa y masiva secundaria a peritonitis lúpica.

ABSTRACT

Objetivo: Evaluar la exactitud diagnóstica de la gradiente albúmina sangre/ascitis (GASA), proteínas totales en líquido ascítico (PTLA), albúmina en líquido ascítico (CAA) e índice de proteínas ascitis/suero (IPAS) para el diagnóstico de ascitis por hipertensión portal. Materiales y métodos: Se realizó un estudio, observacional, retrospectivo, de validez de pruebas diagnósticas. La población estudiada fueron pacientes mayores de 15 años con diagnóstico de ascitis a los cuales se les tomó una muestra para estudio del líquido ascítico mediante la técnica estándar de paracentesis, analizando proteínas totales y albúmina, además de estudio de proteínas totales y albúmina en sangre en el Hospital de Salud Pública Nacional Daniel Alcides Carrión del Callao, Perú (HNDCA), durante el periodo de enero a diciembre del 2012. Se obtuvo la exactitud diagnóstica, sensibilidad, especificidad, VPP y VPN de la gradiente albumina sangre/ascitis (GASA), proteínas totales en líquido ascítico (PTLA), albúmina en líquido ascítico (CAA) e índice de proteínas ascitis/suero (IPAS) para el diagnóstico de ascitis por hipertensión portal o no HTP. Para determinar ascitis por HTP según las pruebas diagnósticas se tomo en cuentas: GASA≥1,1, PTLA<2,5, CAA<1,1 o IPAS<0,5. Resultados: se obtuvieron 126 pacientes con diagnóstico de ascitis a los cuales se excluyó 10 pacientes por tener datos incompletos. De los 116 pacientes finales la edad promedio fue de 53,03 ± 15,73 años, pacientes de sexo masculino fueron 65 (56%) y femenino 51 (44%). Se encontró 61 (52%) líquidos ascíticos debido a HTP por cirrosis hepática, y 55 (48%) de ascitis por NO HTP. La sensibilidad y especificidad para el GASA fue de 93% y 47% respectivamente, para PTLA fue de 80% y 89% respectivamente, para CAA fue de 85% y 87% respectivamente y para el IPAS fue de 83% y 80% respectivamente. El área bajo la curva ROC para el GASA fue de 0,70, de las PTLA fue de 0,84, del IPAS fue de 0,81 y de la CAA fue de 0,86; encontrándose diferencia estadísticamente significativa entre el GASA comparado con los otros tres parámetros (p<0,01). Conclusión: La exactitud diagnóstica de la CAA, PTLA y IPAS es superior a la del GASA para discriminar entre ascitis por HTP o NO HTP, por lo que podrían ser usados en la práctica clínica de forma aislada, o en conjunto para lograr una aproximación diagnóstica más acertada.


Objective: To evaluate the diagnostic accuracy of the Serum-Ascites Albumin Gradient (GASA), Protein Concentration in the Ascitic Fluid (PTLA), Albumin Concentration in the ascitic fluid (CAA) and the Protein Ascites/Serum Ratio (IPAS) for the diagnosis of ascites due to portal hypertension. Materials and methods: it was an observational and retrospective study of validation of diagnostic tests. The study population was patients from a National Public Health Hospital Daniel Alcides Carrion of Callao, Peru, during the period January to December of 2012, patients over 15 years old with a diagnosis of ascites which samples were taken for study by paracentesis with an standard technique, it was analyzed total protein and albumin, as well as study of total protein and albumin in blood. We obtained the diagnostic accuracy, sensitivity, specificity, PPV and NPV of the Serum-Ascites Albumin Gradient (GASA), Protein Concentration in the Ascitic Fluid (PTLA), Albumin Concentration in the ascitic fluid (CAA) and the Protein Ascites/Serum Ratio (IPAS) for the diagnosis of ascites due to portal hypertension. To determine ascites by HTP as diagnostic tests we took into account: GASA ≥ 1.1, PTLA <2.5, CAA <1.1 or IPAS< 0.5. Results: There were 126 patients diagnosed with ascites, 10 patients was excluded for having incomplete data. Of the 116 patients, the average age was 53.03 +/- 15.73 years old, male 65 (56%) and female 51 (44%). 61 (52%) had ascites due to portal hypertension from liver cirrhosis, and 55 (48%) of ascites due to NO HTP. The sensitivity and specificity for GASA was 93% and 47% respectively, for PTLA was 80% and 89% respectively, for CAA was 85% and 87% respectively and for the IPAS was 83% and 80% respectively. The area under the ROC curve for GASA was 0.70, ATPL was 0.84, IPAS was 0.81 and CAA was 0.86, we found statistically significant differences between GASA compared to the other three parameters (p<0.01 ). Conclusion: The diagnostic accuracy of CAA, ATPL and IPAS is higher than the GASA to discriminate between ascites due to HTP or NO HTP, so that they could be used in clinical practice alone or together to achieve a diagnostic approach more successful.

ABSTRACT

OBJECTIVE: To describe the CT and MRI features of hepatic sinusoidal obstruction syndrome (HSOS) caused by herbal medicine Gynura segetum. MATERIALS AND METHODS: The CT and MRI features of 16 consecutive Gynura segetum induced HSOS cases (12 men, 4 women) were analyzed. Eight patients had CT; three patients had MRI, and the remaining five patients had both CT and MRI examinations. Based on their clinical presentations and outcomes, the patients were classified into three categories: mild, moderate, and severe. The severity of the disease was also evaluated radiologically based on the abnormal hepatic patchy enhancement in post-contrast CT or MRI images. RESULTS: Ascites, patchy liver enhancement, and main right hepatic vein narrowing or occlusion were present in all 16 cases. Hepatomegaly and gallbladder wall thickening were present in 14 cases (87.5%, 14/16). Periportal high intensity on T2-weighted images was present in 6 cases (75%, 6/8). Normal liver parenchymal enhancement surrounding the main hepatic vein forming a clover-like sign was observed in 4 cases (25%, 4/16). The extent of patchy liver enhancement was statistically associated with clinical severity classification (kappa = 0.565). CONCLUSION: Ascites, patchy liver enhancement, and the main hepatic veins narrowing were the most frequent signs of herbal medicine induced HSOS. The grade of abnormal patchy liver enhancement was associated with the clinical severity.

The authors conducted a quickly and practice review about ascites, targeting aspects such as definition, etiology, diagnosis, paracentesis, ascitic fluid analysis and management. The goal is to assist healthcare professionals in the early identification of disease and appropriate treatment of patients with ascites, especially cirrhosis.

Este artigo, ao fazer uma revisão da literatura no que tange ao tratamento da ascite no paciente com cirrose, enfatiza a importância da dieta com restrição de sal; do papel da espironolactona no início do tratamento em pacientes com um primeiro episódio de ascite e do tratamento combinado (espironolactona e furosemida) nas ascites recorrentes e da paracentese terapêutica, com reposição de albumina, na ascite tensa. Conclui ressaltando a importância da avaliação do transplante hepático nesta população de doentes.


The present article, reviewing medical literature regarding treatment of ascites in cirrhotic patients, emphasizes the importance of a sodium restricted diet; it also explains the role of espironolactone as the first treatment in a patient with the first episode of ascites, that of the combined treatment with espironolactone and furosemide in cases of recurrent ascites and that of therapeutical paracentesis with albumin replacement in patients with tense ascites. It concludes highlighting the importance of evaluating cirrhotic patients with ascites for liver transplantation.

ABSTRACT

Ascites and dilutional hyponatremia are frequent conditions in cirrhotic patients and its occurrence has a worse prognosis. Thus, patients with ascites should generally be considered for referral for liver transplantation. The present article explores issues related with ascites classification and the treatment corresponding to each of the 3 degrees of ascites. Regarding refractory ascites, recommendations are included on the use of diuretics, large volume paracentesis with the administration of intravenous albumin and use of transjugular intrahepatic portosystemic shunt (TIPS). Finally, physiopathological aspects involved in the development of dilutional hyponatremia and vaptans treatment, are presented, which are aquaretics that selectively blockade vasopressin V2 receptors in the principal cells of the collecting ducts, being one of the most innovative pharmacological interventions for the management of hyponatremia in cirrhotic patients, in the recent years.


La ascitis e hiponatremia dilucional son complicaciones frecuentes en pacientes con cirrosis avanzada y su aparición implica un peor pronóstico. Por ello, los pacientes con ascitis deberían ser referidos para trasplante hepático. El presente artículo explora los aspectos relacionados con la clasificación de la ascitis y el tratamiento correspondiente a cada uno de los 3 grados de ascitis. En relación al manejo de la ascitis refractaria, se incluyen recomendaciones respecto a la eficacia del uso de diuréticos, paracentesis evacuadora radical con reposición de albúmina intravenosa y uso de cortocircuito porto-sistémico intrahepático (TIPS). Finalmente, se abordan los aspectos fisiopatológicos involucrados en el desarrollo de hiponatremia dilucional y el tratamiento con vaptanes, que son acuaréticos que bloquean selectivamente los receptores V2 de vasopresina en las células principales del túbulo colector, constituyendo una de las intervenciones farmacológicas más innovadoras en el manejo de la hiponatremia en cirrosis en los últimos años.

A literatura relata somente alguns casos de recém-nascidos com ascite urinária, geralmente secundária a ruptura de bexiga urinária e não de um cálice renal. Este relato de caso descreve um caso raro de ascite urinária em recém-nascido com válvula de uretra posterior que teve ruptura de um cálice renal, neste caso diagnosticada, até onde sabemos, pela primeira vez por ultrassonografia.


The literature reports only rare cases of urinary ascites, usually secondary to rupture of the bladder and not to renal calyx rupture. The present report describes an uncommon case of posterior urethral valve in a newborn with ascites caused by the rupture of a renal calyx that, to our knowledge, was diagnosed for the first time by ultrasonography.

ABSTRACT

Lymphocytic ascites with low serum-ascites albumin gradient (SAAG) are observed mainly in tuberculous peritonitis, peritoneal carcinomatosis, and pancreatic disease. However, pelvic inflammatory disease (PID) induced generalized peritonitis causing diffuse ascites has been rarely described. We report a 26-year old female patient, who was diagnosed as generalized peritonitis with diffuse ascites due to Chlamydia trachomatis infection. Gynecologic examination did not show the clue of PID and in the analysis of ascites, low SAAG, predominant lymphocyte count and high level of adenosine deaminase were noted. Although the best impression was tuberculous peritonitis on the base of these findings, the laparoscopic finding was consistent with PID and the PCR for C. trachomatis infection in cervical swab was positive. This case suggests that C. trachomatis peritonitis should be considered as a rare cause of low SAAG and lymphocytic ascites in sexually active women and should be intensively evaluated including laparoscopic examination.

A ascite é a mais comum das três complicações da cirrose. A sua presença é indicativo de mau prognóstico, e está associada a complicações que aumentam a morbidade e a mortalidade. O gradiente de albumina sérico-ascítico é o método utilizado para se fazer o diagnóstico diferencial da ascite associada à hipertensão portal daquela de outra etiologia. A patogênese da ascite cirrótica revela primordialmente dois fatores: alterações na hemodinâmica portal e retenção de sódio e água. A teoria que explica a formação da ascite e as alterações hemodinâmicas na cirrose é a da vasodilatação. Esta vasodilatação é associada às altas concentrações de substâncias como o óxido nítrico dentre outras. O tratamento da ascite consiste na restrição de sódio, diuréticos orais, paracentese abdominal, derivação peritoneovenosa, TIPS e transplante de fígado.


Ascites is the most common of the three major complications of cirrhosis (the others complications are hepatic encephalopathy and visceral haemorrhage). Its presence is an indicative sign of poor prognosis, predisposing the patient to many complications which increase morbidity and death rate. The serum-ascites albumin gradient is the method which you can differentiate ascites caused by portal hypertension from others etiologies. In ascites pathogenesis there are two factors to be considered: The portal hemodynamic and the sodium and water retainers. The theory which explains the ascites formation and the hemodynamics alterations in cirrhosis is the vasodilatation one. The vasodilatation would be secondary to high serum concentrations of vasodilators, including nitric oxide and others. The treatment of ascites in cirrhotic patients includes clinical and surgical procedures like saline restriction, oral diuretics, abdominal paracentesis, peritoneovenousderivation, TIPS, and liver transplantation.
